The City of Walnut only processes the DS-11 application form.
Appointment times are available the following times through the appointment system linked below:
Mondays:
8:00 a.m. - 4:00 p.m.
Tuesdays:
8:00 a.m. - 12:00 p.m.
2:00 p.m. - 4:00 p.m. (walk-in service available)
Wednesdays:
10:00 a.m. - 4:00 p.m.
Please read all of the instructions below before your appointment to avoid rejection of your application.

NOTE: Along with presenting the following ORIGINAL documents, you must also submit a COPY of each document with your application. Copies must be on 8 1/2 x 11 paper, with a separate sheet required for each document. All copies must be single sides (DO NOT copy back to back). A separate set of copies of parent's I.D. are required for EACH minor child application.

IDENTIFICATION
Driver's License/State ID (DO NOT copy back to back)
Student I.D. (age 16 or age 17) (DO NOT copy back to back)
Requirements:
Original Live Birth Certificate or Original Naturalization Certificate
Completed, but not signed, Passport Application
Photo Identification (i.e. Driver's License or California ID)
One Color Photograph (2x2 inches)
Only Checks will be accepted for payment (Two checks will be required per each applicant)
All applicants, including children, must be present
Fees:
Passport Books, Adults 16 & up : $130 payable to U.S. Department of State and $35 payable to City of Walnut
Passport Cards, Adults 16 & up: $30 payable to U.S. Department of State and $35 payable to City of Walnut
Passport Books, Children 15 & under: $100 payable to U.S. Department of State and $35 payable to the City of Walnut
Passport Cards, Children 15 & under: $15 payable to U.S. Department of State and $35 payable to the City of Walnut
Additional fees are required for expedited & overnight services. These fees will be added to your payment to the U.S. Department of State.
No CREDIT CARDS or DEBIT CARDS accepted.
Children applying for a passport that are under 16 years of age, MUST be accompanied by both parents. Applicants between the ages of 16-17 MUST be accompanied by at least one parent.
Passport Cards can ONLY be used by land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean.
